Par-Q - Physical Activity Readiness Questionnaire - This is a questionnaire for individuals to find out more about their physical ability. This will be used by AceHealth trainers to determine the safety or possible risk of exercising for an individual.
Fitness Assessment Form - Completing this form before starting personal training is important for a number of reasons. First, the results will highlight your current health & fitness status. Second, trainers use those results to determine their their plan of action. Finally, this data will be used to measure progress.
